This form is the overall entry form for all teams. We would be grateful if you could return it as soon as possible. Also, we will only guarantee to accept the first 32 entries we receive after which teams may be placed on a waiting list. Each participant is also asked to complete an individual form and a waiver of liability agreement. All forms must be returned by Friday December 4th, 1998, at the latest.
